Application for Enrolment
Please complete an application form (one for each child) and return with requested supporting documents. At the bottom of this page is an editable PDF, so you can either fill it out electronically and submit via email to baldivisgardens.ps.enrolments@education.wa.edu.au, or print a hard copy to fill out and return to the office. If needed, you can also collect a hard copy from the office.
You will also be required to send through a copy of your child’s birth certificate and some proof of address i.e. utility bill (e.g. gas, water, energy), lease agreement, rates etc.
We accept scanned or photographed evidence of these supporting documents with the enrolment application.
Note: We do not need an AIR Immunisation History Statement with an enrolment application. It is collected as an up-to-date record (not more than 2 months old) when you receive an enrolment acceptance notification, accept the place and are finalising your child’s Enrolment.

Enrolment
Once an offer of a place has been received from Baldivis Gardens Primary School, an official enrolment form and other forms will be sent to you to fill out, along with a request to submit further supporting documentation. See list of requirements below:
Forms you will receive:
- Enrolment Form
- Student Health Care Summary
- Additional Information (Kindergarten only)
- Parental Consent Form
Supporting documents:
- Immunisation Statement (no more than 2 months old)
If applicable:
- Additional health care documents (if child has a diagnosis)
- Visa documentation
- Copy of child’s Passport
- Family court order documents

Parenting Plans
The School Education Act 1999 requires parents to keep us informed of any parenting plans, Family Court orders or other orders which apply to your children. These are important information for us and are treated in the strictest confidence. If there any orders the school should be made aware of, please mention this when completing the enrolment forms.
Student Details Update
Once enrolled please ensure your child’s student details are up to date on our system. If you have changed your address, email or phone numbers then we need to know. It is very important in an emergency or when your child is sick that we have your correct contact details. Our funding system also relies on accurate student enrolment information being entered to ensure we are resourced correctly.
